Registration and Enrollment

District 148 offers two ways to register your child: online registration and in person.  Please click the  Home Page link and see the "Online Registration" link at the bottom of the page to register online.

To register in person please go to Roosevelt Elementary School located at 111 w. 146th Street, Monday - Thursday from 8:00 a.m. to 12:00 p.m. in our Main Office (summer hours only).  Please ask for Ms. Garrett, Clerk.

  • All parents or legal guardians must show proof of residency requirements within our attendance boundaries.  Acceptable forms of proof are a mortgage bill, rental agreement or utility bills (e.g. People's Gas, Com Ed, Water Bill) that state your current address.
  • Parents or legal guardians must show a driver's license, state identification card, or passport.
  • Students must have an official birth certificate with the seal.  Copies are not accepted.
  • Students must have proof of current medical immunizations.
  • All students are required to be re-registered every year.  If you no longer live within the attendance boundaries and have no District 148 approved exceptions, the student may not re-register at the school.
